Overview
We welcome you to this training platform.
As part of the ITMAV program, this training aims to equip CWs and managers to identify, support and refer older adults in vulnerable situations or at risk of frailty in order to improve their connection or reconnection with the relevant resources in their community and to contribute to the enhancement of their well-being, the realization of their autonomy, as well as the maintenance of their active participation in the community.
This training includes 10 modules. We invite you to follow them in order, as each module unlocks upon the completion of the previous one. Inside each module, you will find:
- the description of the theme addressed, the objectives pursued and its approximate duration;
- one or more audioclips which contain the main learnings;
- downloadable/printable PDF of those presentations, offered in two formats (2 or 3 slides per page);
- a quiz to consolidate your learning;
- the complementary material, including downloadable tools and links to specific resources on the explored theme;
- a list of the references used.

In several modules, social isolation is used as a typical example to introduce content to support CWs and managers in their work with older adults in vulnerable situations and at risk of frailty. Social isolation has a major impact on the health of older adults. Community work in the ITMAV program is carried out in multiple contexts, including, but not limited to, social isolation.
Periodic updates will be made to better reflect changes to the ITMAV program over the years.

How will this training help me in my daily life?
The objectives of this training are to:
- Allow you to equip yourself in order to better identify, support, and refer older adults in vulnerable situations or at risk of frailty, in order to improve their connection or reconnection with the relevant resources in their community and to contribute to the enhancement of their well-being, the realization of their autonomy, as well as the maintenance of their active participation in the community;
- Enhance understanding of the roles, limits, and responsibilities of CWs;
- Optimize the effectiveness and efficiency of CWs;
- Develop and adapt the CWs' approaches to meet the needs of older adults.
At the end of the training, you will be able to:
- Explain the phenomenon of social isolation among older adults;
- Name different strategies to identify, support, and refer older adults in vulnerable situations;
- Apply elements from this training to concrete situations;
- Identify challenges encountered in your work and ways to overcome them;
- Know how to work with key stakeholders;
- Know how to position yourself towards complex situations.
